For First Time Clients

If you have enrolled in (purchased) an individual or couple/family session,

 Complete the Client Information form and the Session Agreement & Consent

for each person.

OR

If you have enrolled in (purchased) a Space Clearing Session

Complete the Space Clearing Questionnaire.
